COVID-19 Office Updates (11/18/21)

We are slowly returning to normal, but there are still certain protocols that we have in place.

  • Everyone is required to wear a mask, so please make sure that you have yours with you.
  • The office doors are open and you are welcome to come in and check in at the front desk. If you feel more comfortable waiting in your car, you may call in on your cell phone and we can review your information over the phone.
  • We are limiting the number of patients in the office so we ask that you come alone to the appointment. If you do need a driver, we would ask that your driver remain in the car. Children may be accompanied by one parent or guardian. We ask that all siblings remain at home.
  • Please bring your medical insurance card and a list of current medications, including dosage amounts.
  • If you are a contact lens wearer, please wear them to your appointment and bring your glasses with you.
  • We will be asking you about Covid-19 symptoms and exposure. If you do start experiencing symptoms or have been exposed to anyone with symptoms, we would ask that you reschedule for a minimum of 14 days from exposure/symptoms. Thank you for your cooperation.
